Patty N. Funck

“I have a passion for working with older adults and their families. I have experienced care issues with my parents and in-laws, including Alzheimer’s and mental health issues. I hope to ease the burden for other families by providing guidance and a caring heart.”

pattydadhorizontalPatty has worked as a social worker in Lubbock for more than 30 years, and owned her own business, Noble Care Solutions, LLC, since 1996. Patty is a licensed Clinical Social Worker, and obtained her Master in Social Work from the Worden School of Social Services at Our Lady of the Lake University, and a Bachelor Of Arts in Social Welfare from Texas Tech University.

Patty’s company, Noble Care Solutions, offers care management services, provides medical social services for local Home Health agencies, and performs assessments for Lubbock Meals on a Wheels. Patty’s business expanded into care management in 1999, and she is a member of the National Association of Geriatric Care Managers, now called the Aging Life Care Association. Her background includes a wide range of experience in the medical field, beginning as a hospital social worker and transitioning into home health and care management. She currently specializes in working with aging adults with cognitive disorders, particularly dementia and Alzheimer’s disease.

Natalia Jackson

Natalia Jackson has been a Licensed Clinical Social Worker in Texas since 2002. She received her Master Degree in Social Work from Louisiana State University, and has a Master Degree in Religion from the University of Chicago. She has experience doing therapeutic work with family systems and has also worked in hospitals doing complex discharge planning. Her hospital experience involved work in skilled nursing units, rehabilitation, and geriatric psychiatric unit. Natalia is committed to the well-being of her clients and enjoys getting to know clients’ families as well.

Rachel Storrs

Rachel is from Lubbock, Texas and attended Texas Tech University and graduated with a Bachelor’s degree in Social Work in 2005. She also attended Texas State University in San Marcos and in 2010 earned a Master’s Social Work Degree. Rachel has 15 years of experience working in the medical field including hospital, nursing home & home healthcare settings. She enjoys working with clients that are older aged or have disabilities with the goal of helping them live the highest quality of life they can. Rachel enjoys the outdoors, gardening, home decor and spending time with family & friends.
